The National Survey of Parents of Chronically Ill Children
If you are parenting a child with any type of chronic illness please consider participating in this survey
The purpose of this survey is to better understand the difficulties and needs of individuals who are parenting chronically ill children.
Findings from the survey will be used to expand and improve educational training programs for parents who face the many challenges presented by their child’s chronic illness. You are encouraged to participate if your child has any type of chronic medical condition or developmental disorder, and your child is no older than 18 years of age.
The survey should take 10 to 15 minutes to complete. Your participation is entirely confidential and we are not requesting any information that could be used to identify you. To thank you for your participation, we have compiled a Resource Guide to web sites and books for parents of chronically ill children. You can download the Resource Guide after completing the survey.
